To sign up for FSB Shallowater Online, please complete one of the following applications and mail them back to us.

Please do not submit more than one application per account.    

Select your password. Desired password" needs to be at least 5 alpha/numeric characters long. Be sure to remember your password. You will be required to use your password each time you access your account.  Your password cannot be the last 5 digits of your social security number or phone number.
  
You will not be able to use your FSB Shallowater Online Account until you have received confirmation from the bank via email that your account has been set up. You should receive confirmation within 24 to 48 hours after submitting your application.
